North Tees and Hartlepool NHS FT partners were amongst the first, nationally, to pilot the benefits of offering care for patients, safely and in their own homes, avoiding the disruption and risk of prolonged hospital stays.
We were asked to co-design a service for frail, elderly patients and have been delivering a "virtual ward" service since 2023, providing up to 100 virtual beds, seven days a week across Hartlepool and Stockton.
We are continually looking to expand and improve our joint, GP=led service, which works with a highly skilled team of practitioners, who visit patients out in the community and work together with a multi-disciplinary team approach.
