Hartlepool & Stockton Health, along with the University Hospital of North Tees and Hartlepool Foundation Trust and North East Ambulance Service, are part of an Alliance Partnership formed to design, launch and run new Integrated Urgent Care Centres based in the hospital sites in Stockton and Hartlepool. These centres provide GP out of Hours, Walk-in and Minor Injuries support for the local population.
A team of Healthcare Assistants, Nurses, Minor Injuries specialists and Advanced Practitioners (Paramedics) are clinically led by one of our team of 67 GP’s, on site 24 hours a day, 365 days a year in both the Hartlepool and Stockton centres. We encourage people who need support to call the NHS 111 Service, where one of the team can book you an appointment with the right professional, in the right place. Launched on 1st April 2017, in our first 2 months we saw 19,000 local people and are delighted with the feedback so far! “I just wanted to express to you my amazement at how fantastic your Doctor really is!” Patient comment

On 1st April 2017 the GP Federation began to offer more accessible appointments with local GP’s and other healthcare professionals from 3 locations every evening and weekend. Evening appointments run from 6.30pm to 8pm on all sites, with weekend clinics 10am – 1pm and 2pm – 5pm on Saturdays, and 11am – 1pm and 2pm – 4pm on Sundays.

You can access an appointment with a nurse or GP, as well as healthcare assistants who can help with health checks, weight management or dressings. At the weekend you can speak to a pharmacist who can help you to understand your medications, and you can also see a Physiotherapist who can put together short term plans to help less chronic conditions. Our team of 26 GPs, 24 Nurses, 22 Receptionists, 10 Health Care Assistants, 3 Pharmacists, 3 Physiotherapists and 1 Coordinator have offered over 8,000 additional appointments in the first 3 months of operation. If an appointment outside of traditional surgery hours would benefit you or someone you care for, please book via your practice. The World Health Organisation defines health as "a state of complete physical, mental, and social well-being and not merely the absence of disease or infirmity." At Hartlepool & Stockton Health we are leading a pilot to provide extra, holistic support to patients who are living with long term clinical conditions which may put them at risk of frequent, unplanned hospital admissions.
Often known as “Social Prescribing”, our Care Coordination service helps patients to focus on the important social and mental factors that affect their health. Our 20 Care Coordinators work with every surgery in Hartlepool and Stockton, helping over 100 patients each week to create their own simple plan to enable them to feel better. We work with over 40 community and voluntary sector partners to help local people to stay safely and happily in their own homes. In a recent survey 27% of patients said that they felt the service was life changing and 69% felt their circumstances had vastly improved. We are please to share with you all that the Care Coordinator Team have been recommissioned for a further 2 years. 
On 1st August we began a new Clinical Assessment and Peer Review service across Hartlepool, Stockton and Darlington. The service acts as a second pair of eyes check, by accessing the latest clinical guidance, when a GP feels that a patient may need an appointment with a specialist. During the 1 year pilot the CASPeR service will either send on the referral within 48 hours, or recommend another way to manage conditions. We believe we can reduce any variation in local healthcare provision, so that all patients across the area get prompt access to the right specialist in the right place, and have the right tests to make sure that any treatment needed is done quickly.
The CCG have asked the H&SH to take on a further 4 specialties, Geriatric Medicine, Respiratory, Neurology and General Surgery. You will all have access to the new templates and pathways.
