Our mission at H&SH is to improve the health and wellbeing of people living locally, and we try to do this in a number of ways. We are a GP Federation, which means that we are proud to have every practice across Hartlepool and Stockton as our shareholders and members. We support our practices as they deliver access and continuity for their patients, and we help their Primary Care Networks who work as groups of practices together across neighbourhoods of around 50,000 people. We deliver larger services ourselves across the bigger footprint of Hartlepool and Stockton, and even Tees Valley, and we pride ourselves on our high standards. Finally, we have strong relationships with a number of other local teams, organisations, and charities, both in healthcare and in related sectors, working together with the aim of improving population health. We bring our vision to life by delivering high quality services and by creating a culture where our brilliant team feel valued for the role they play in innovating, improving standards and reducing the inequalities that can exist in how people access healthcare. |
